Person centred services

Individual tailored services are at the heart of everything we do. Every client is unique, with different needs and preferences. This means:

  •  tailoring support to your individual needs
  •  ensuring you have access to information, advocacy and advice to make informed decisions about your care and support
  •  finding new collaborative ways of working that support service users to actively engage in the design, delivery and evaluation of the services they receive
  • developing local partnerships to provide a range of services for people to choose from, and opportunities for social inclusion and community development
  • developing the right leadership and organisational systems to enable staff to work in creative, person-centred ways
  • embedding early intervention, reablement and prevention so service users are supported in a way that is right for them
  • recognising and supporting carers in their role while enabling them to maintain a life beyond their caring responsibilities
  • ensuring that all service users have access to other community services and resources, and enabling them to benefit from them
